What to Expect at Your First Acupuncture Visit

August 20, 2024 · Dr. Kevin Tai

Starting something new can feel daunting — especially when it involves needles. But most patients leave their first acupuncture session saying some version of the same thing: "That wasn't what I expected at all."

Here is what you can actually expect.

Before You Arrive

Eat a light meal one to two hours before your appointment. Coming in on a completely empty stomach can make you feel lightheaded during or after treatment. Wear loose, comfortable clothing that gives easy access to your arms, legs, and lower back.

If you are taking any medications or supplements, bring a list. This helps your practitioner understand your full picture.

The Intake

Your first session will be longer than follow-up visits — typically 75 to 90 minutes. We spend a significant portion of that time just talking.

Your practitioner will ask about your primary concern, but also about seemingly unrelated things: your sleep quality, your digestion, whether you tend to run hot or cold, what your stress levels look like, and more. In Traditional Chinese Medicine, the whole body is interconnected, and these details matter for an accurate diagnosis.

We will also examine your tongue and take your pulse — not just to check your heart rate, but to gather information about the state of your internal organs and energy systems. This is one of the most distinctive aspects of TCM diagnosis.

The Treatment

Based on the intake, your practitioner will select a set of acupuncture points tailored specifically to you. You will lie comfortably on a treatment table — usually face up, though sometimes face down or on your side.

The needles are extremely thin — about the width of a human hair. Insertion is quick, and most patients feel very little discomfort. You may notice a sensation of heaviness, warmth, or a dull ache at certain points; in TCM, this is called "de qi" and is considered a positive sign that the point is activated.

Once the needles are placed, you will rest for 20 to 30 minutes. Many patients doze off.

After Your Session

It is common to feel deeply relaxed — sometimes pleasantly groggy — immediately after acupuncture. Drink plenty of water and take it easy for the rest of the day if you can.

Some patients notice improvement right away; others find that changes emerge gradually over the following 24 to 48 hours. Occasionally, symptoms feel slightly more intense for a day before improving — this is a normal part of the body's response to treatment.

What to Ask

Come with questions. Your practitioner wants to know what you are hoping to get out of treatment, what you have already tried, and what matters most to you. The more we understand your goals, the more effectively we can work toward them together.
